I have also never seen that. Is this all happening on the same song, or multiple songs?

it is in multiple songs, but they are probably derived from the same song...
It's most odd and quite random.

Well, then it's likely your source song is corrupt, and it's affected the others. You should look to create a new project from a logic template and import your tracks, etc and see if that takes care of it.
